Discharge Process:

Your healthcare team will work closely with you to ensure a smooth and informed transition from hospital to home, rehabilitation, or a step-down facility. Discharge planning is an ongoing process, and various members of your care team will provide guidance and support throughout your stay.
Before You Leave, Please Confirm the Following with Your Care Team:

Wound care instructions, if applicable

Prescribed medications

Please note: Due to legal regulations, the hospital pharmacy is only permitted to dispense medication to in-hospital patients.
You are welcome to fill your prescription at Clicks Pharmacy in Menlyn Maine or at a pharmacy of your choice.
What to Take Home With You:
​Ensure you take the following items when you are discharged:

Radiology reports and scans

Orthopaedic appliances (e.g. crutches, braces)

Electronic devices and chargers

All personal belongings
