DISCHARGE AND FEEDBACK
 

Discharge:
 

  • Your healthcare team will work closely with you to ensure you have the information and resources to continue your care and recovery seamlessly.  Various healthcare team members will discuss your discharge instructions throughout your stay.

Confirm the following with your health team: 
 

  • Your follow-up appointment.

  • Your wound care, if necessary. 

  • Prescribed medication - Due to legislation, our Pharmacy only serves “in - hospital patients”.

  • You are welcome to make use of the link to Clicks Pharmacy in the mall or a Pharmacy of your choice.

Make sure that you take the following with you:
 

  • Radiology reports and scans

  • Orthopaedic appliances, such as crutches

  • Electronic appliances and cables

  • Personal items

  • Whether you are going home, to a rehabilitation or a step-down facility, we strive to ensure a smooth transition of care between the hospital and your destination.
